For efficient processing of your referral, please fax the following information:

• Patient Demographics:
  Include the patient’s full name, date of birth, address, and contact details.

• Insurance Information:
  Provide details such as the insurance provider, policy number, and group number.

• Diagnosis:
  Please include a detailed diagnosis along with chart notes and any relevant images.

STAT/URGENT Referrals: If you are submitting a STAT referral, please fax the referral to our office and call us immediately after the fax is successfully sent. This will ensure prompt scheduling for your patient.
